Exam Request Form
The deadline to fill out the Exam Request Form for final exams has passed. The deadline was February 02 at 11:59 PM. All exam requests for finals made after this time must be made through our secure late fee submission form. The $15 late fee must be paid prior to your exam date and time, or your exam will not be administered and you will receive a zero.
Failure to take a proctored examination will result in failure of the course, regardless of the average of other grades.
